Please note that there is a fee of 1,000.00 pesos for each additional deceased individual and 1,680.00 pesos for each additional Engraving of vault cover.

Requirements for Inurnment:

  1. Death Certificate (photograph or digital copy of the Original Document)
  2. Right of Perpetual Use (RPU) Certificate issued by MVC or Receipt of full Payment (photograph or digital copy of the Original Document)
  3. Exhumation Permit (photograph or digital copy of the Original Document)
  4. Cremation Certificate (photograph or digital copy of the Original Document)
  5. Authorization letter if bearer of URN is not vault owner, duly signed by vault owner (Digital Photocopy)
  6. Valid Government issued ID of Vault Owner and Authorized person (photograph or digital copy of the Original Document)
  7. Deceased's Photo (Image size must have 1:1 ratio (image's width and height are equal, creating a square)

Note:

  • Original copy to be presented or shown before the inurnment takes place.
  • Size of urn should have a maximum of 6" width & 9" height only
  • Minimum of 3-day notice before inurnment schedule
  • Engraving - (7 working days)

  1. Over the Counter (OTC) -Validated Deposit Slip
  2. GCash, PayMaya, Bayad Center Apps -Confirmation / Transaction Receipt
